Vendor Fees $50 Fee PER 10 x 10 spot. Payment due upon submitting application.
No space will be confirmed without payment in full. Please remit payment at Village Hall via check, cash, or credit card (transaction fees may apply). 

Please note we will only accommodate one vendor per product, these registrations will be based on first paid registration received. No Direct sales will be allowed, looking for handcrafted, home made products, etc. Please make all checks payable to The Village of Minooka, 121 McEvilly Road, Minooka IL. 60447

NO REFUNDS WILL BE GIVEN. SPECIFIC SPACES CANNOT BE GUARANTEED.

Contract specifications:

• Set up can commence no earlier than 9 a.m.
• Adhere to the event start time of 1 p.m. and remain set up until 8 p.m.
• Must provide my own tents, tables, chairs, lighting and heating.
• Tents are to be secured by weights.
• Not block entrances to local businesses or walk ways with your displays.
• Clean area and dispose of trash properly.
• Failure to adhere to rules and regulations will prohibit your return.
• 5 amps and one outlet of electric is provided. Must bring your own cords.
• If more power is needing please advice asap.
• You may have generator, but we ask that you bring a quite generator

WAIVER AND RELEASE OF ALL CLAIMS
Directions: Please read this "Waiver and Release of all Claims" Carefully.
Understand that if you elect to sign this "Waiver and Release of All Claims", You acknowledge that you are knowingly and voluntarily giving up certain rights that you may
have, either individually or on behalf of your minor child or ward.
I understand that anytime I participate in activities like the one(s) I am registering to participate in by signing this form there are certain risks of physical injury, or damages that may arise. I fully understand that, while participating, I may be injured as a result any number of circumstances, including my own physical conditions or carelessness, the carelessness of others, conditions result in from inclement weather, slips and falls, premises defects, inadequate or defective equipment, inadequate supervision or instruction, and any other circumstances inherent in indoor or outdoor activities.
With full understanding of these risks, I voluntarily agree to assume the full risk of injury, damage, or loss resulting from my participation in any and all activities I am registering for. Therefore, I agree to waive and relinquish all claims that I might have, as a result of my participation, against the Village of Minooka, Its officials, agents, volunteers, employees, contractors, and cooperating governmental units.
If I am signing on behalf of my minor child or ward, I further agree to waive and relinquish any and all claims that may arise on his or her behalf as a result of participating in these actives, additionally, I waive and relinquish all claims I might have as a result of my minor child's or ward's participation in these activities.
I acknowledge that I have read and fully understand all of the terms of this "Waiver and Release of All Claims" and that I am voluntarily signing this form.
In addition, I understand that The Village of Minooka may, through its employees, agents, or contractors, photograph or video record the participants in the recreational programs and activities it offers. Such images may be used in publications, broadcasts, and on the internet to inform the public about the Village of Minooka's activities. I agree that in the event that I, or my minor child or ward, is photographed or recorded in this way, any images so produced may be used by the Village of Minooka as describe, and I expressly authorized such use.
